By completing this form I confirm I am the legal guardian and I give permission for my child to use The Hive gym facilities independently, including during periods when they may not be under the direct supervision of a coach or staff member. I understand that gym training involves inherent risks, including risks associated with free weights, barbells, strength equipment, cardiovascular equipment and physical exercise. I understand that independent access does not mean that The Hive will continuously supervise the member while they train. I confirm that I have discussed the conditions of independent access with my child and agree that they must follow The Hive's safety instructions, and facility rules. I understand that The Hive may restrict, suspend or withdraw independent access if it considers this necessary for the safety of the member or others.
